From what I see Pharma is allowed to proceed as with current strain change and

Follow-up cranial CT scan carried out one day later revealed the persistence of the mass lesions and the progression of hydrocephalus. Another chest roentgenogram showed diffuse opacity throughout both lungs with a diffuse brochopneumonic appearance.

Despite the institution of intravenous amphotericin B, the patient continued to deteriorate neurologically and died two days after beginning therapy. The autopsy showed diffuse Cryptococcus pneumonia and multiple intracerebral lesions with numerous fungal cells. Cultures revealed C. gattii.

 

DISCUSSION

Cryptococcus gattii is emerging as a primary human and animal pathogen. The principal source of the organism is wood debris in hollows, particularly that of eucalyptus8 and is prevalent in the tropical and subtropical areas and rare in cold climates5. In contrast, C. neoformans is widespread in the environment, especially in areas burdened with bird excreta, and has a worldwide distribution. After gaining entry through the respiratory tract, both have trophism for the central nervous system (CNS); patients infected by C. gattii are more likely to evidence focal pulmonary and CNS mass2,7,9. Both inhibition of polymorphonuclear leukocyte migration to the site of infection3 and inhibition of neutrophil function11 by products of C. gattii may promote survival of extracellular organisms, and local multiplication to form cryptococcomas.

Infection due to C. gattii can pose a diagnostic challenge to clinicians. This case vividly illustrates the fact that, pulmonary cryptococcosis with mass-like lesion with associated cerebral infection, exhibit radiologic features that mimic those of lung cancer metastatic to the CNS. Isolated pulmonary cryptococcosis simulate pulmonary cancer particularly when it appears as an apical mass, including with a typical Pancoast's syndrome6. In these cases, cryptococcal antigen testing by latex agglutination, a rapid test with high specificity, would be useful in the differential diagnosis.

Fungemia10 and funguria4 by C. gattii is highly unusual. On the other hand, infections due to C. neoformans can be isolated from blood up to 63% of patients9. Positive blood culture is a sign of very poor prognosis and most patients had a high tissue burden of organisms in the lung and CNS1, like our patient.

In summary, C. gattii must be included in the differential diagnosis of pulmonary and brain masses. Furthermore, with fungal cultures, smears, and serologic test systemic cryptococcosis will be diagnosed sooner, leading to earlier treatment which may be life-saving.

Scientists at the UNC School of Pharmacy led by Tony Hickey, Ph.D.,
 
 
vetted a dry powder vaccine provided by Harvard University that is administered using an inhaler. Hickey's group specializes in developing drugs and vaccines that can be inhaled as a dry powder.
 
 
The vaccine used in the study was a Bacillus Calmette-Guérin (BCG) vaccine, which is not common in the United States but is used extensively throughout the world.
 
 
 
Moreover, Hickey is a co-founder of Oriel Therapeutics, a company developing dry-powder inhaler products to effectively deliver medicines to the lungs to treat a wide range of respiratory diseases, such as asthma and chronic obstructive pulmonary disease.
 
 
He is also the founder, president and CEO of Cirrus Pharmaceuticals. Both companies are located in North Carolina's Research Triangle Park.

...prosecutions of pharmaceutical company executives under the Park doctrine of strict liability- a result that members of Congress, including Senator Grassley, have been seeking.

The Park doctrine provides that management can be held criminally liable for a company's violations of the federal Food, Drug, and Cosmetic Act (FDCA) even if management was unaware of the violation. A March 4, 2010, Wall Street Journal article discussing the GAO report cited an unnamed FDA official in support of this conclusion:
A FDA official said the agency has the authority to prosecute corporate executives for criminal actions within their companies under a provision called "strict liability." He said the government doesn't have to show intent to defraud in order to get a conviction. He added that the provision is an important tool that hasn't been used much in recent years.

The World Health Organization will attempt Tuesday to dial back

undergo to fully adapt to humans.

"The science is not there yet," said Adolfo Garcia-Sastre, a microbiologist

Studies could be done that would chart a sort of roadmap for adaptation,
he suggested, but researchers would first need to find an animal that
could act as a surrogate for human infection. While it's believed ferrets
could be that model, testing would be needed to validate that.

Then viruses would have to be manipulated in order to try to determine
what role each of a flu virus's eight genes play in transmissibility.


"It's not quick," Garcia-Sastre said, noting this work would have to be
done in high biocontainment laboratories.

He wasn't willing to make an on-the-record prediction of how near or far
H5N1 is from becoming a virus that easily infects people, though he
privately named a figure that is significantly higher than Osterhaus's
guess.

"But it is complete speculation. So don't quote me," Garcia-Sastre said.

now chairs the health committee at the Council of Europe, claimed major firms organised a

"campaign of panic" to put pressure on the World Health Organisation (WHO) to declare a

"false pandemic" to sell vaccines. Dr Wodarg said the WHO's "false pandemic" flu campaign

is "one of the greatest medicine scandals of the century." He said that the "false

pandemic" campaign began last May in Mexico City, when a hundred or so "normal"

reported influenza cases were declared to be the beginning of a threatening new

pandemic, although he said there was little scientific evidence for this.

Nevertheless he argued that the WHO, "in cooperation with some big pharmaceutical

companies and their scientists, re-defined pandemics," removing the statement that "an

enormous amount of people have contracted the illness or died" from its existing definition

and replacing it by stating simply that there has to be a virus, spreading beyond borders

and to which people have no immunity.[78] The WHO has responded by stating that it

took its duty to provide independent advice seriously and guarded against interference

from outside interests. Announcing a review of its actions, WHO spokeswoman Fadela

Chaib stated that, "Criticism is part of an outbreak cycle. We expect and indeed welcome

criticism and the chance to discuss it."[79][80]

In 1602, angelica
 
was introduced in Niort, which had just been ravaged by the plague,
 
 
and it has been popular there ever since. It is used to flavour liqueurs or aquavits (e.g. Chartreuse, Benedictine, Vermouth and Dubonnet), omelettes and trout, and as jam. The long bright green stems are also candied and used as decoration.
Angelica is unique amongst the Umbelliferae for its pervading aromatic odour, a pleasant perfume entirely different from Fennel, Parsley, Anise, Caraway or Chervil. One old writer compares it to Musk, others liken it to Juniper.
 
 
Even the roots are fragrant, and form one of the principal aromatics of European growth - the other parts of the plant have the same flavour, but their active principles are considered more perishable. Angelica contains a variety of chemicals which have been shown to have medicinal properties. Chewing on angelica or drinking tea brewed from it will cause local anesthesia, but it will heighten the consumer's immune system.
 
 
It has been shown to be effective against various bacteria, fungal infections and even viral

The essential oil of the roots of Angelica archangelica contains β-terebangelene, C10H16, and other terpenes; the oil of the seeds also contains β-terebangelene, together with methylethylacetic acid and hydroxymyristic acid.
Angelica seeds and angelica roots are sometimes used in making absinthe.
 
 
 
A seeds of a Persian spice plant known as Golpar (Heracleum persicum) are often erroneously labeled as "angelica seeds." True angelica seeds are rarely available from spice dealers.

What is NTM?
 
 
"NTM" is short for      Nontuberculous        Mycobacteria. 
 
 
 
 
NTM lung disease is a devastating chronic illness mostly affecting women.  
 
 
Where does it come from?
 
 
NTM bacteria exist naturally in our environment.  People contract NTM lung disease from
 
exposure to the bacteria, possibly through activities like showering and gardening.  NTM
 
lung disease is not contagious.  In the shower and when working with soil, tiny NTM
 
particles become aerosolized and we inhale them.  People who are predisposed to
 
developing an infection from those inhaled NTMs get sick,  due to underlying physical
 
reasons we don’t totally understand yet.
 
 
 
What is it like to have NTM?
 
Once a person has NTM they are often left with permanent lung damage and recurring
 
illness for the rest of their lives.  Productive members of society are struck down with a

Mycobacterial species, other than M. tuberculosis and M. leprae, may be referred to as nontuberculous mycobacteria (NTM), although controversy exists over this designation. In the past, they variously have been referred to as atypical, anonymous, environmental or unclassified. MOTT, the acronym for Mycobacteria Other Than Tuberculosis, may also be used to describe these microbes.
 
 

NTM are a heterogenous group of bacilli that are generally found in the environment (water and soil). Unlike the organisms that cause tuberculosis and leprosy, NTM are thought to be contracted from environmental sources and not from infected humans. Most are saprophytic,
 
 
 
but some NTM are pathogens which may cause severe disease or even death.

NTM differ from the classic tubercle bacilli in several aspects.
NTM have a wider temperature range for growth,
including temperatures not found in the human body,
and their growth rates tend to be more variable than M. tuberculosis.
 
 
 
 
Unlike tuberculosis, which is inherently drug susceptible,
wild strains of NTM have varying patterns of natural drug resistance,

typically manifesting resistance to isoniazid and pyrazinomide with varying
susceptibility to rifampin, ethambutol and streptomycin.

 
 
 
 
 
This heterogenous resistance makes therapy far less predictable

than with disease caused by M. tuberculosis.
 
 
 
 
 
Infections from NTM

Lung infections from NTM range in severity from rapidly progressive, destructive pneumonic disease to indolent disorders, with minimal physical manifestations evident in the short term. While these infections frequently are associated with pre-existing disease or trauma, it is important to emphasize these individuals do not have apparent defects of immunity.
 
 
 
Rather they have mechanical or other functional disturbances of the airways and lungs, which promote susceptibility to the infections. The underlying lung disorders coupled with the organisms' heterogenous resistance to drugs complicates treatment protocols greatly; this makes a standard approach to treatment problematic.
 
 
 
 
On the Rise
..........................

Although it is unclear why, human infections due to NTM appear to be increasing at a significant rate across the United States. These increases, which occurred before the appearance of AIDS, are not due to the impact of human immunodeficiency virus infection. Interestingly, this overall increase includes a growing number of individuals who do not have clear predisposing lung disorders or factors.

 
 
In addition, a disproportionate number of cases seem to occur among women, and more cases than in the past are involving young people. At National Jewish Health, we employ early diagnosis and aggressive medical treatment as the bulwarks of our program to control the infection and assure life expectancy and quality of life.
 
 
 
This upsurge may indicate a change in the ecosystem and the distribution of mycobacteria in our water supply. In fact, tentative evidence supports Legionnaire's disease as a model to study the spread of NTM. As is thought to be the case with NTM, legionella bacteria are not spread from person-to-person but are generally inhaled from a water source.

A research team led by epidemiologists from the National Institute of Allergy and Infectious Diseases, part of the National Institutes of Health, analyzed hospital discharge records of patients in 11 states whose combined total population represents 42 percent of the country. They reviewed database records spanning 1998 to 2005, and identified more than 16,475 hospitalizations associated with pulmonary NTM in people without AIDS.

Of the 11 states studied, Florida, New York and California had 62 percent of the pulmonary NTM hospitalizations. The scientists chose these states to compare trends by geographic area. They found the annual prevalence of disease increased significantly among men and women in Florida (3.2 percent per year for men and 6.5 percent per year for women) and among women in New York (4.6 percent per year).
 
 
 
There were no significant changes in California. Whether these geographic differences in prevalence are a result of exposure to NTM, or increased concentrations of mycobacterium in certain environments, or both, is not clear. However, previous studies have found high disease prevalence in the southeastern United States, particularly along the coastal regions of the Atlantic and Gulf coasts.

    is that human cases of H5N1 contracted from birds are continuing to increase. Indeed, confirmed cases for 2006 are running ahead of those for last year. But the difference is slight; 97 worldwide for all of last year versus 111 through the end of November 2006. This difference could be entirely explained by better surveillance. Moreover, the real concern is not sporadic bird-to-human transmission, but human-to-human transmission. Far more people die of tuberculosis in an hour than all those known to have died from H5N1.

    Production Steps
    Bulk production begins with the cultivation of the virus in a fermenter equipped with numerous process parameters to control temperature, pH, dissolved oxygen, and other factors. Two methods of mass cultivation of cells are recognized in the industry today, microcarrier cultures and free-cell suspension cultures. Both systems begin cultivation of the cell line in a fermenter, which can be scaled up to thousands of liters.

    In microcarrier systems, the cells are first anchored to microbeads. In the presence of nutrient media, the cells grow and proliferate covering the beads uniformly. Microbeads provide a high surface-area-to-volume ratio, which can lead to high cell densities. Once a bead is covered, the cells are dislodged, dispersed, and allowed to reattach to achieve another round of cell growth on the surface of the bead.

    Larger volumes of media are required to achieve the same results with free-cell suspension because the cell line proliferates while growing freely suspended in the nutrient medium. However, the scaling up of the system is easier, and there is no limit to the volume.

    Formulation is the process of mixing the product setting dose requirements, concentrations, and final volumes, and establishing secondary process parameters, such as materials, vials, syringes, caps, plugs, incubators, and so on. Filling, often referred to as sterile filling, is part of formulation. Filling involves bringing together sterile vials or syringes with sterile filtered vaccine solution. The syringe or vial is filled in a controlled environment to ensure that the final product is sterile. Packaging is also an important step for ensuring the integrity of the product for the duration of its shelf life.

    Cell Line Selection
    The cell line used to cultivate the virus must be able to propagate the virus in large quantities, must be rapid and efficient in expressing the desired virus, and must be suitable for a wide variety of flu strains. It is desirable that the cell line be able to grow in a chemically defined synthetic medium that does not contain animal-derived components. It should also be scalable for industrial processes. Equally important, if the cell line has not been previously approved by regulatory agencies, the requirements for licensing should be known and validated.

    Serum-Based Media

    Serum-based media have some disadvantages. First, typical acceptance criteria for serum can vary as much as ?20 percent, which could contribute to batch-to-batch variations during fermentation.     A further major source of toxicity in the case of the flu vaccines are the adjuvants,
    substances added in order to boost the immunogenicity of the vaccines. There is a
    large literature on the toxicities of adjuvants.
     
     
     
    Most flu vaccines contain dangerous
    levels of mercury in the form of thimerosal, a deadly preservative 50 times more
    toxic than mercury itself [9]. At high enough doses, it can cause long-term
    immune, sensory, neurological, motor, and behavioural dysfunctions. Also
    associated with mercury poisoning are autism, attention deficit disorder, multiple
    sclerosis, and speech and language deficiencies.
     
     
    The Institute of Medicine has
    warned that infants, children, and pregnant women should not be injected with
    thimerosal, yet the majority of flu shots contain 25 micrograms of it.
    Another common adjuvant is alum or aluminium hydroxide, which can cause
    vaccine allergy, anaphylaxis, and macrophage myofascitis, a chronic inflammation
    syndrome, In cats, alum also gives rise to fibrosarcomas at the site of injection
    [15].
     
     
     
    Numerous new adjuvants are no better, and could be worse. According to a
    recent review in a science and business pharmaceutical publication [15], most
    newer adjuvants including MF59, ISCOMS, QS21, AS02, and AS04 have

    "Although the virus has not delivered any devastating surprises, we have seen some surprises on other fronts," Chan said in her prepared remarks. "We anticipated problems in producing enough vaccine fast enough, and this did indeed happen. But we did not anticipate that people would decide not be vaccinated."


    Chan did not refer explicitly to those allegations in her speech. But, given the vast range of health information now available to the public, she said, public health leaders may not have the authority and credibility they once did.


    "People make their own decisions about what information to trust, and base their actions on those decisions," she said. "The days when health officials could issue advice, based on the very best medical and scientific data, and expect populations to comply, may be fading. It may no longer be sufficient to say that a vaccine is safe, or testing complied with all regulatory standards, or a risk is real."

    Novartis profits rise as it issues warning to nations
     
     
     
    Swiss pharmaceutical company Novartis today announced an 8% increase in profits from
     
    2008 to 2009, driven largely by sales of H1N1 vaccine, Agence France-Presse (AFP)
     
    reported. But during the announcement in Basel, outgoing CEO Daniel Vasella warned that
     
    the company could retaliate against governments now trying to cancel flu-shot contracts.
     
    Countries reneging on payment for vaccines that have already been delivered will not
     
    receive a priority response during the next pandemic, he said.

    An inquiry heard yesterday that the WHO allegedly softened its criteria for declaring a
     
    H1N1 flu pandemic last spring - just weeks before announcing there was a worldwide
     
    outbreak.

     
     
    Critics said the decision was driven by pharmaceutical companies desperate to recoup the
     
    billions of pounds they had invested in researching and developing pandemic vaccines
