Trump-Pence budget would weaken response to outbreaks of infectious disease
By Sean Cahill, opinion contributor - 05/30/17 01:21 PM EDT If the Trump-Pence budget released earlier this week had been in place just a few short years ago, the level of public health in Austin, Ind., would likely now mirror that of a country in sub-Saharan Africa, where rates of HIV transmission and infection are high and there are little to no public health prevention services available. In 2015, Austin, which is located in rural Scott County, was hit with a devastating outbreak of HIV that will cost taxpayers at least $180 million to treat. At its height, then-President Obama was receiving daily updates on attempts to contain the epidemic, which began in November 2014 when the first of 181 HIV diagnoses were made throughout a 12-month period. For context, a total of five cases of HIV had been diagnosed in all of Scott County during the previous decade. Four months after the initial HIV diagnosis, then-Gov. Mike Pence declared a public health emergency, and the U.S. Centers for Disease Control and Prevention oversaw a contact tracing investigation to find — as quickly as possible — anyone who had had sexual contact or shared intravenous needles with the first 11 people who had been diagnosed with HIV. The labor-intensive process, which is described in detail in a New England Journal of Medicine article about the HIV outbreak, required a team of experienced epidemiologists who were trained in conducting sensitive interviews about highly personal behavior. Through these interviews, investigators identified 536 people who might have been exposed to HIV. They located 468 of them and administered HIV tests on the spot. If the Trump-Pence budget had been in place, this work would not have been possible. The proposed budget would cut CDC funding by $1 billion, or 9 percent, and the cuts specifically target programs that respond to outbreaks of infectious disease, such as HIV. (The Trump-Pence budget would also cut the CDC’s HIV, hepatitis, sexually-transmitted disease, and tuberculosis prevention by 17 percent). As a result of the CDC’s contact tracing efforts, an additional 170 people with HIV were identified, and they required immediate medical care. The Midwest AIDS Education and Training Center, one of many such centers around the country that are funded by the Ryan White HIV/AIDS Program, took the lead in connecting these people with healthcare providers. The center also provided in-depth training in HIV care to doctors and other clinicians who had never before treated anyone with HIV, which is a complicated chronic disease that impacts treatment for other, more common, chronic conditions such as diabetes, cardiovascular disease, and hypertension. If the Trump-Pence budget had been in place, this work would not have been possible, as the Trump-Pence budget proposes total elimination of every AIDS Education and Training Center in the country. Treatment of HIV is medically-intensive. Fortunately, just weeks after the first case of HIV was diagnosed, as the authors of the NEJM article write, the state of Indiana had “fortuitously” accepted the Medicaid expansion offered by the Affordable Care Act (ACA). This new availability of health insurance to people who had, for the most part, been uninsured, “helped to ensure health care coverage in the largely uninsured and impoverished community that was affected by the outbreak and facilitated the immediate enrollment, coverage, and access to critical health care services, including HIV treatment.” If the Trump-Pence budget had been in place, this treatment would not have been given. The budget proposes a massive cut in Medicaid services which, when coupled with passage of the American Health Care Act, would almost surely result in a loss of health insurance — and access to care — for those now being treated for HIV. People with HIV have disproportionately benefited from state Medicaid expansions. Before passage of the ACA, 36 percent of people living with HIV obtained their health insurance through Medicaid; after passage of the ACA that number rose to 42 percent. Meanwhile, the rate of no insurance among people with HIV fell from 22 percent to 15 percent. The most amazing thing about the Indiana HIV outbreak — which rapidly spread through a loosely connected network of opioid addicts who were sharing needles — was the public response to it. It required coordination among federal, state, and county agencies. It required extensive lobbying of Pence by federal and state officials, who needed the then-governor to back a new state law in Indiana that would legalize needle exchange programs. It required a massive effort to put public health, education, and prevention resources in place where there had been none previously. Indeed, just one year before the first HIV diagnosis, the only healthcare provider in Scott County that offered free HIV testing and substance abuse treatment was a Planned Parenthood clinic that was forced to close after Indiana lawmakers voted to defund it. The second most amazing thing about the Indiana HIV outbreak is how quickly lawmakers like Pence seem to have forgotten about it. The CDC has identified 220 counties in 26 states with conditions that make them just as vulnerable to an eruption of HIV as Scott County was: pervasive opioid addiction among impoverished rural residents where there is no access to sterile needle exchanges or treatment for addiction and limited access to basic healthcare and HIV prevention services. Imagine if the public had turned its back on the residents of Austin as HIV made its way through the population? Imagine if HIV had been allowed to progress among those newly infected without any treatment? Imagine what Austin would look like now? Well, that’s easy. Austin now has an HIV prevalence rate of approximately four percent, which is comparable to that found Nigeria and Cameroon, where HIV spreads among poor people who have limited access to treatment or prevention services. Unlike Nigeria and Cameroon, however, when a major public health crisis struck Austin, its residents enjoyed the benefit of a coordinated local, state, and federal response to HIV that continues today. The Trump-Pence budget is a radical rejection of the social compact that has sustained the United States for 50 years. Its proposed gutting of Medicaid would end the provision of health insurance for millions of poor people that has been in place Lyndon Johnson’s War on Poverty in 1965. Its cuts to the Children’s Health Insurance Program and general welfare programs that help people living in poverty pay for food would decimate the social safety net. It casts our most vulnerable people aside and leaves them to fend for themselves without the resources they need to succeed. If enacted, vast swaths of our country would look more like a developing nation than our own. Well, also the corrupt, incompetent politicians (House and Senate) who support him! It's all rather remarkable....the Republicans are strongest in rural states that are most at risk for effects of climate change (torrential rains & flooding, drought, agricultural collapse) and these areas are also suffering from poor health and high levels of narcotic addiction. So what does Drumpf do? Propose cutting their Medicaid, food subsidies, funding for drug programs etc. The idiot voters of this country are like lemmings, marching off of a cliff! I can understand the anxiety about gun ownership (I own a few), and the Gorsuch appointment pretty much takes care of that for another 20 years or so. I have started a new thread about the biggest problem in America now. The ultra-left who will not give up and are trying to assassinate the president. The budget must be cut and balanced and if not - the government will not only run out of money - but not be able to borrow any more and shut down. The Democrats passed much of this without ever thinking of anything but votes by many people who do not work, collect on these programs, especially illegal immigrants, and now are freaking out because they can't continue to spend.
If we do not increase our defense spending and cut welfare to people who not even citizens of our country and people who will not work and don't even try, we will face another great depression. What you had most to fear when Trump took the oath were the haters, the violent, and other nations who want to continue ripping the U.S. off, taking the jobs, selling us products, and shutting down our factories. Had Hillary been elected, there would have been a no-fly zone in Syria, she would order a nuclear counter strike when Russia launched and most of use would be dead. What was done for the last eight years was criminal. Huge sums of money were paid in cash to the Iranians to finance terrorism. We have this ridiculous investigation when our President Obama was openly helping the very extremists and enemies of American while being raised by Muslims and was their greatest friend. Trump is no friend of Russia. They hate him. So get your ducks in a row and realize who the real enemies and terrorists are that are the most danger to America. It has nothing to do with Trump. It is the anarchists and racists that put any past racial unrest or hatred of color to shame. You are going to see a lot more problems from them than Trump. And much like all the movie stars and has beens, you may get your wish and he may be assassinated. That will be a sad day for America and there were be a civil war and anarchy in streets so bad - you will wish people had just accepted Trump and tried to help in whatever way they could. A truly horrific abuse of the system, but not stupid. Actually Chump is amazingly canny.
Take the fake news for instance: He both publishes fake news (like the Time magazine cover) and then accuses the media of fake news. That way he promotes his own image fantastically well. Then, having found that Russia was tinkering with the electoral system (to get the Democrats out of power) he publicly blames the Democrats for not stopping it - because they were in power at the time (even though they started an attempt to remove such threats but did not have time or the support to finish the programme). Of course he is not going to push on with the Obama administration's unfinished attempts to block such things, they are far too useful to him. He both gets help AND can blame the opposition for the unfairness - a double win. I am no Democrat, I think them quite objectionally crooked. But, compared to Trump's administration a corkscrew would look straight!

I read about it and it wasn't on Drudge and if Wasseman Schultz has broken the law then prosecute her but as of right now the Chief and her are just arguing over interpretation of some rules if the Chief is violating the rules there should be consequences if not then he has NOTHING to worry about does he? tempest in a teapot unless other evidence comes forward |

Whichever of the two turns out to have been in the wrong (and it is not usually the cop in cases like this so I suspect the Democrat is in the wrong - lets hope the truth comes out soon.) the public can find out through the media. Silence the media and we will never know. That would be the greater crime. I have, in my past, had many dealings with TV news and newspapers. They were irritating, intrusive and not adverse to twisting the facts till they squeaked. But, even the worst of them, told no outright lies. That vice was exclusively perpetrated by a few politicians. At their worst, the media is a necessary evil, at their best, they are the life blood of politics. No swamp was ever drained in the dark.

Say it like it is Arirish! That is why I am such a yankophile! I too come from a family where many fought and died for our empire and for the right to speak our mind. Apart from the fact that our governments have managed, quite successfully, to disarm us and turn us into sheep over the last few decades. We still occasionally manage to overcome their, often oppressive, activities. No one in any society should be in a position where they are allowed, by consent or default, to have absolute control, especially religious and political leaders. Defend the right to bear arms and to question the ruling elite (Yes, us rebellious Anglo-Irish can use that word too). Silencing people on a thread is as heinous and aggressive as silencing the press.

I'll never address you by your first name and I don’t consider myself your peer and he said OK.” Normal people, normal politicians do not speak to one another like this. Conway made sure to say he's a great boss for women. Sure thing, Kellyanne. Just keep yourself at arms-length. Seriously. “I do think that it's important to set up that level of deference and humility when you’ve got someone who’s your boss. Now the President of the United States, now the chief of staff, a general, a secretary, who is clearly your senior, you know, your superior” she said. Conway continued, "I think it's very important that people not treat elected officials [like the President and Vice President] as their peers." “Don’t read into that, everybody,” Conway added. “Stop. Put your keyboards down, that’s not about anyone.” Inherently there is respect for the office of the president, but during Bill Clinton's presidency, conservatives, the media and Republicans alike smashed that respect to smithereens. Why any White House official would recount a story about how honored they are to shine Trump's shoes is beyond me Well he is not stupid enough to be Tomen, nasty enough to be Joffrey or feminine enough to be Cersei.
