In the USA it is required that all Influenza associated pediatric deaths be reported.  Given all the modern communications it was a little surprising to find this in the recent CDC weekly flu report:

Quote Eleven influenza-associated pediatric deaths occurring in previous influenza seasons were reported to CDC during week 35. Three deaths occurred in the 2008-09 influenza season, six deaths occurred in the 2009-10 season, and two deaths occurred during the 2011-12 season bringing the total number of deaths reported during those seasons to 136, 288, and 37 respectively.

Some information took 6 years to be reported from hospitals etc to CDC Unhappy...I guess someone had time to go through old files.

While the numbers are not high they do represent an error of between 2% and 5.4%, which does sound like a high margin of error for what is supposed to be a well regulated system
