I understand they have to reform the reform... I just hope it's middle class friendly.
Relative to the Senate bill, health reform lowers premiums for families with income below $44,000 and above $66,000.
??? what about this income range? 45,000 to 66,000 out in the cold?
( perhaps this is the major fatted calf income range? :)
Relative to the House bill, reform makes premiums less expensive for families with income
between roughly $55,000 and $88,000.
The question is What got passed? ... as I see it, they voted on the Senate version?
Senate version looks odd in comparison.

...More than a dozen GOP attorneys general are determined to challenge the requirement in federal court as unconstitutional.
Starting in 2014, the new law will require nearly all Americans to have health insurance
through an employer, a government program or by buying it directly. That year, new
insurance markets will open for business, health plans will be required to accept all
applicants and tax credits will start flowing to millions of people, helping them pay the
premiums.
Those who continue to go without coverage will have to pay a penalty to the IRS, except in
cases of financial hardship. Fines vary by income and family size. For example, a single
person making $45,000 would pay an extra $1,125 in taxes when the penalty is fully
phased in, in 2016.

...the new law is loaded with provisions that will place many health care providers under further pressure and scrutiny. Among them are new limitations on physician-owned hospitals, new Stark self-referral requirements for referrals for imaging services; a requirement for formal compliance programs; and expanded resources for fraud and abuse enforcement.
Although portions of the law are subject to court challenge, those provisions affecting providers likely will not be included in those challenges. Fairfax, VA: Before the medical journal Neurology printed a study alerting consumers to
the high levels of zinc found in denture adhesives, there was not a lot of information
available about the potential for denture cream zinc poisoning.
Dr. Sharon P. Nations of the Department of Neurology, University of Texas Southwestern
Medical Center, worked with a team of researchers to identify four patients with various
neurological abnormalities. The common denominator was that all four patients wore
dentures and used excessive amounts of denture adhesive.
Zinc concentrations ranging from about 17,000 to 34,000 µg/g were identified in Fixodent
and Poli-Grip denture creams. When the four patients stopped using denture cream, three
experienced a drop in serum zinc levels and two reported mild neurologic improvement due
to copper supplementation. No alternative source of excess zinc ingestion or explanation
for hypocupremia was identified.
The study concluded that denture cream contains zinc and that chronic excessive use may
result in hypocupremia and serious neurologic disease.
When ABC's "Good Morning America" caught wind of the study this past spring, zinc
poisoning and its link to denture cream adhesives suddenly went mainstream.
"Good Morning America" revealed that 34 million Americans wear dentures, a third of
whom use denture adhesive cream. To most, denture cream is not unlike other hygienic
products like toothpaste or shampoo, which require no warnings against excessive use. As
a result,
most people are unaware of the health risks associated with denture creams.

Nothing shocked me more in the early days of caring for my elderly mother than the
discovery that Medicare, the U.S.'s universal health coverage for those ages 65 and older,
does not pay for so many of the things the frail elderly require.

Average lifetime cancer risk from exposure to diesel exhaust alone may exceed 1 in 100,000 and could be as high as 1 in 1,000.156 Inhalation of particulate matter from diesel exhaust is classified by EPA as a likely human carcinogen,156 and by IARC as a probable human carcinogen for lung cancer.157 Diesel exhaust particles usually consist of an elemental carbon core surrounded by organic matter and other substances, including sulfuric acid, that adhere to it once airborne and are small enough to be inhaled into the alveolar regions of the lung.77 In 2009, EPA recommended that owners of buildings, including schools, constructed or renovated between 1950 and 1978, test masonry and window caulking for high PCB levels.135 The chemicals were mixed into caulking to make it rubbery when applied to interior and exterior building surfaces. As the caulking ages, however, it can disintegrate into PCB-containing particles and vapors that can fall to the ground or other surfaces and infiltrate building ventilation systems.

Many of the inert ingredients in pesticides also are toxic, but are not required to be tested for causing chronic diseases such as cancer. For example, xylene is used as the inert ingredient in almost 900 pesticides222 and has been associated with increased risk of brain tumors, rectal cancer, and leukemia.223
Asbestos is classified by IARC as a lung and laryngeal carcinogen, and some evidence suggests it may increase risk for non-Hodgkin lymphoma, chronic lymphocytic leukemia, and multiple myeloma.78 One meeting speaker noted that the World Health Organization, the World Bank Group, international labor organizations, and numerous public health scientists and policymakers have urged a global ban on asbestos. Some countries (e.g., Brazil) have banned asbestos, but its use continues in many nations, including the United States. Perchloroethylene and Trichloroethylene Perchloroethylene (PCE, also known as perc and tetrachloroethylene) is a solvent that has been a mainstay of the dry cleaning industry for decades. It is classified as “reasonably anticipated to be a carcinogen” by the National Toxicology Program (NTP).77 Approximately 28,000 dry cleaners in the U.S. use perc. Dry cleaning workers who inhale PCE are at risk for liver damage and neurological problems. Some large industrial and commercial dry cleaners emit more than 10 tons of PCE into the atmosphere each year. The public also has been exposed to PCE due to improper disposal that has contaminated soil and drinking water at hundreds of locations across the country.142 High levels of PCE in drinking water are associated with elevated breast cancer risk.143 Animals exposed to high levels of PCE developed kidney and liver tumors.144 Dry cleaning businesses reduced PCE emissions by more than half between 1996 and 2006 by replacing old machinery and improving efficiency. Some have begun using alternative cleaning methods that do not require PCE. The industry, however, has strongly resisted a ban on the chemical. EPA’s most recent amendments (2008) to regulations on the use of PCE by dry cleaners require dry cleaners located in residential buildings (typically the smallest establishments) to phase out perc use by 2020. Larger freestanding and industrial/commercial dry cleaners are required to upgrade equipment to detect and reduce PCE emissions, but are not required to cease using the chemical.145 Trichloroethylene (TCE) is classified by IARC as probably carcinogenic to humans (Group 2A)146 and as “reasonably anticipated to be carcinogenic to humans” by NTP.77 A review of recent studies found evidence that TCE is strongly associated with kidney, liver, and biliary cancers, and is a suspected carcinogen for cervical cancer, Hodgkin and non-Hodgkin lymphomas, and leukemia.147 Occupational exposures are greatest among workers involved in metal degreasing and the manufacture of adhesives, paint removers, varnishes, paints, lacquers, typewriter correction fluids, printing inks, and spot removers. TCE previously was used as a dry cleaning agent. Children's exposure to particulate air pollution is of special concern because of their greater vulnerability to toxics of all kinds. In 2008, USA Today published a series of articles152 based on its study that used EPA’s model to track the path of industrial pollution and mapped the locations of nearly 128,000 schools to determine the levels of toxic chemicals near schools. Academic researchers who partnered with USA Today to conduct the study found that 20,000 schools—about one in six—are within a half-mile of a major industrial plant. Arginine (abbreviated as Arg or R)[1] is an α-amino acid. The L-form is one of the 20 most common natural amino acids. At level of molecular genetics, in the structure of the messenger ribonucleic acid mRNA, CGU, CGC, CGA, CGG, AGA, and AGG, are the triplets of nucleotid bases or codons that codify for arginine during protein synthesis. In mammals, arginine is classified as a semiessential or conditionally essential amino acid, depending on the developmental stage and health status of the individual.[2] Infants are unable to meet their requirements and thus arginine is nutritionally essential for infants.[3] Arginine was first isolated from a lupin seedling extract in 1886 by the Swiss chemist Ernst Schultze.

The amino acid side chain of arginine consists of a 3-carbon aliphatic straight chain, the distal end of which is capped by a complex guanidinium group.
With a pKa of 12.48, the guanidinium group is positively charged in neutral, acidic and even most basic environments, and thus imparts basic chemical properties to arginine. Because of the conjugation between the double bond and the nitrogen lone pairs, the positive charge is delocalized, enabling the formation of multiple H-bonds.
Arginine is a conditionally nonessential amino acid, meaning most of the time it can be manufactured by the human body, and does not need to be obtained directly through the diet. The biosynthetic pathway however does not produce sufficient arginine, and some must still be consumed through diet. Individuals who have poor nutrition or certain physical conditions may be advised to increase their intake of foods containing arginine. Arginine is found in a wide variety of foods, including[4]:
Arginine is synthesized from citrulline by the sequential action of the cytosolic enzymes argininosuccinate synthetase (ASS) and argininosuccinate lyase (ASL). This is energetically costly, as the synthesis of each molecule of argininosuccinate requires hydrolysis of adenosine triphosphate (ATP) to adenosine monophosphate (AMP); i.e., two ATP equivalents.

The pathways linking arginine, glutamine, and proline are bidirectional. Thus, the net utilization or production of these amino acids is highly dependent on cell type and developmental stage.
On a whole-body basis, synthesis of arginine occurs principally via the intestinal–renal axis, wherein epithelial cells of the small intestine, which produce citrulline primarily from glutamine and glutamate, collaborate with the proximal tubule cells of the kidney, which extract citrulline from the circulation and convert it to arginine, which is returned to the circulation. Consequently, impairment of small bowel or renal function can reduce endogenous arginine synthesis, thereby increasing the dietary requirement.
Synthesis of arginine from citrulline also occurs at a low level in many other cells, and cellular capacity for arginine synthesis can be markedly increased under circumstances that also induce iNOS. Thus, citrulline, a coproduct of the NOS-catalyzed reaction, can be recycled to arginine in a pathway known as the citrulline-NO or arginine-citrulline pathway. This is demonstrated by the fact that in many cell types, citrulline can substitute for arginine to some degree in supporting NO synthesis. However, recycling is not quantitative because citrulline accumulates along with nitrate and nitrite, the stable end-products of NO, in NO-producing cells.[5]
Arginine plays an important role in cell division, the healing of wounds, removing ammonia from the body, immune function, and the release of hormones.[2][6][7] Arginine,[8] taken in combination with proanthocyanidins[9] or yohimbine[10], has also been used as a treatment for erectile dysfunction.

The distributing basics of the moderate structure found in geometry, charge distribution and ability to form multiple H-bonds make arginine ideal for binding negatively charged groups. For this reason, arginine prefers to be on the outside of the proteins where it can interact with the polar environment. Incorporated in proteins, arginine can also be converted to citrulline by PAD enzymes. In addition, arginine can be methylated by protein methyltransferases.
Arginine is the immediate precursor of NO, urea, ornithine and agmatine; is necessary for the synthesis of creatine; and can also be used for the synthesis of polyamines (mainly through ornithine and to a lesser degree through agmatine), citrulline, and glutamate. For being a precursor of NO, (relaxes blood vessels), arginine is used in many conditions where vasodilation is required.[2] The presence of asymmetric dimethylarginine (ADMA), a close relative, inhibits the nitric oxide reaction; therefore, ADMA is considered a marker for vascular disease, just as L-arginine is considered a sign of a healthy endothelium.

A clinical trial found that patients taking an L-arginine supplement following a heart attack didn't improve in their vascular tone or their hearts' ability to pump. In fact, six patients who were taking L-arginine died than those taking a placebo and the study was stopped early with the recommendation the supplement not be used by heart attack patients.[14][15][16] The supplement is still widely marketed.
The Mayo Clinic web page on L-arginine reports that inhalation of L-arginine can increase lung inflammation and worsen asthma.[17]
Arginine may boost the production of growth hormone[18], although this has not been proven.[citation needed]
