I find hard to belive that these things happen....still!!!! 

Note to myself....

Always make sure if you having a procedure that you see its a new implement that is being used......

Just like if you in a foreign country,never accept a opened bottle, make sure it's opened in front of you, or do it yourself......

We just had 2 people leave a ship without going into quarantine......tracked now in isolation.....

Mistakes will occur just had to make sure  SOP are adhered to.....
