What is a vaccine-derived poliovirus?

A vaccine-derived poliovirus (VDPV) is a strain related to the weakened live poliovirus contained in oral polio vaccine (OPV). If allowed to circulate in under- or unimmunized populations for long enough, or replicate in an immunodeficient individual, the weakened virus can revert to a form that causes illness and paralysis.

OPV is a safe and effective vaccine that contains a combination of one, two, or three strains of live, weakened poliovirus, and is given in the form of oral drops. OPV has been instrumental in eradicating wild polioviruses around the world, including in the United States, because it stops the spread of the virus by inducing immunity in the gut. VDPVs emerge when not enough people are vaccinated against polio, and the weakened strain of the poliovirus from OPV spreads among under-immunized populations.

The United States has used inactivated poliovirus vaccine (IPV) exclusively since 2000. IPV is given as an injection in the leg or arm, depending on the person’s age and protects against paralytic disease caused by any type of poliovirus, including VDPV.

New York State Department of Health informed the Orange County Department of Health that analysis from the CDC detected the polio virus in wastewater samples taken from June and July in two geographically different locations in Orange County.

These environmental findings – which further indicate potential community spread – in addition to the paralytic polio case identified among a Rockland County resident, underscore the urgency of every New York adult and child getting immunized against polio, especially those in the greater New York metropolitan area.

As part of ongoing surveillance efforts, New York wastewater samples are shared with the Global Polio Laboratory Network (GPLN), which includes CDC and the World Health Organization (WHO). GPLN confirmed that the case in New York is genetically linked to two Sabin-like type 2 (SL2) isolates, collected from the early June samples from Rockland County and samples from greater Jerusalem, Israel as well as to the recently-detected VDPV2 from environmental samples in London, UK. New Yorkers should know that this does not imply that the individual case identified in Rockland County, New York has travel history to Israel or the UK.

NYSDOH will continue to aggressively assess the spread of the virus and communicate openly with New Yorkers.

The polio virus was identified in wastewater collected from June and July in two locations in Orange County, indicating the virus could be circulating in the community. Orange County health officials said Thursday there were no confirmed cases in their suburban and rural county.

New Yorkers should know that the latest environmental findings do not indicate that the individual identified in Rockland County was the source of the transmission, and case investigation into the origin of the virus is ongoing. NYSDOH continues to work with global, national, and local public health authorities to aggressively assess the spread of the virus and ensure prevention measures, particularly immunization clinics, are in place – as the best way to keep New York polio-free is to maintain high immunity across the population through vaccination.

All New Yorkers who are unvaccinated, including children by 2 months of age, those who are pregnant, and people who have not completed their polio vaccine series previously should get immunized right away. Unvaccinated New Yorkers who live, work, go to school in, or visit Rockland County, Orange County, and the greater New York metropolitan area are at the highest risk of exposure.

The term "poliomyelitis" is used to identify the disease caused by any of the three serotypes of poliovirus. Two basic patterns of polio infection are described: a minor illness which does not involve the central nervous system (CNS), sometimes called abortive poliomyelitis, and a major illness involving the CNS, which may be paralytic or nonparalytic.[12] Adults are more likely to develop symptoms, including severe symptoms, than children.[1]

In most people with a normal immune system, a poliovirus infection is asymptomatic. In about 25% of cases, the infection produces minor symptoms which may include sore throat and low fever.[3] These symptoms are temporary and full recovery occurs within one or two weeks.[1][3]

In about 1 percent of infections the virus can migrate from the gastrointestinal tract into the central nervous system (CNS).[1] Most patients with CNS involvement develop nonparalytic aseptic meningitis, with symptoms of headache, neck, back, abdominal and extremity pain, fever, vomiting, stomach pain, lethargy, and irritability.[13][14][15] About one to five in 1000 cases progress to paralytic disease, in which the muscles become weak, floppy and poorly controlled, and, finally, completely paralyzed; this condition is known as acute flaccid paralysis.[16] The weakness most often involves the legs, but may less commonly involve the muscles of the head, neck, and diaphragm.[1] Depending on the site of paralysis, paralytic poliomyelitis is classified as spinal, bulbar, or bulbospinal. In those who develop paralysis, between 2 and 10 percent die as the paralysis affects the breathing muscles.[3]

Encephalitis, an infection of the brain tissue itself, can occur in rare cases, and is usually restricted to infants. It is characterized by confusion, changes in mental status, headaches, fever, and, less commonly, seizures and spastic paralysis.[17]

DJ...In most cases symptoms may be missing at all (72%) or mild (24%) ...so it will be very easy to believe it is CoViD...CoViD itself may result in weaker immunity so the polio-vaccine-virus may have more chances...

Afghanistan and Pakistan still use those (sugar lumb) weakened live polio virus vaccines...Most children do not mind eating some sugar (with the vaccine on it...). 

Poliomyelitis is caused by infection with a member of the genus Enterovirus known as poliovirus (PV). This group of RNA viruses colonize the gastrointestinal tract[18] – specifically the oropharynx and the intestine. The incubation time (form the first signs and symptoms) ranges from three to 35 days, with a more common span of six to 20 days.[1] PV infects and causes disease in humans alone.[19] Its structure is quite simple, composed of a single (+) sense RNA genome enclosed in a protein shell called a capsid.[19] In addition to protecting the virus' genetic material, the capsid proteins enable poliovirus to infect certain types of cells. Three serotypes of poliovirus have been identified – poliovirus type 1 (PV1), type 2 (PV2), and type 3 (PV3) – each with a slightly different capsid protein.[20] All three are extremely virulent and produce the same disease symptoms.[19] PV1 is the most commonly encountered form, and the one most closely associated with paralysis.[21]

Individuals who are exposed to the virus, either through infection or by immunization via polio vaccine, develop immunity. In immune individuals, IgA antibodies against poliovirus are present in the tonsils and gastrointestinal tract and able to block virus replicationIgG and IgM antibodies against PV can prevent the spread of the virus to motor neurons of the central nervous system.[22] Infection or vaccination with one serotype of poliovirus does not provide immunity against the other serotypes, and full immunity requires exposure to each serotype.[22]

A rare condition with a similar presentation, nonpoliovirus poliomyelitis, may result from infections with nonpoliovirus enteroviruses.[23]

Poliomyelitis is highly contagious via the fecal–oral (intestinal source) and the oral–oral (oropharyngeal source) routes.[22] In endemic areas, wild polioviruses can infect virtually the entire human population.[24] It is seasonal in temperate climates, with peak transmission occurring in summer and autumn.[22] These seasonal differences are far less pronounced in tropical areas.[24] The time between first exposure and first symptoms, known as the incubation period, is usually 6 to 20 days, with a maximum range of 3 to 35 days.[25] Virus particles are excreted in the feces for several weeks following initial infection.[25] The disease is transmitted primarily via the fecal–oral route, by ingesting contaminated food or water. It is occasionally transmitted via the oral–oral route,[21] a mode especially visible in areas with good sanitation and hygiene.[22] Polio is most infectious between 7 and 10 days before and after the appearance of symptoms, but transmission is possible as long as the virus remains in the saliva or feces.[21]

Factors that increase the risk of polio infection or affect the severity of the disease include immune deficiency,[26] malnutrition,[27] physical activity immediately following the onset of paralysis,[28] skeletal muscle injury due to injection of vaccines or therapeutic agents,[29] and pregnancy.[30] Although the virus can cross the maternal-fetal barrier during pregnancy, the fetus does not appear to be affected by either maternal infection or polio vaccination.[31] Maternal antibodies also cross the placenta, providing passive immunity that protects the infant from polio infection during the first few months of life.[32]

DJ...so indications are Polio may be spreading north of New York City...earlier "positive Polio sewage samples" showed up in London-UK and Jerusalem-Israel...

There may be a link to people not able or willing to get any vaccinations...eventhough the Polio-vaccine-with weakened but still alive polio-virus may be behind it. 

This polio-story may become more alarming when more polio-cases start showing up...However the mix of CoViD-pandemic and polio-virus is "not welcome"...

DJ-I think more sewage samples and at random testing blood/stool for the polio-virus may give better indications on spread. Good information and communication (getting the good info to the right people !) would be very welcome...

Since CoViD-symptoms and Polio-symptoms may look similar early intervention can make a difference. 

There is no cure for polio, but there are treatments. The focus of modern treatment has been on providing relief of symptoms, speeding recovery and preventing complications. Supportive measures include antibiotics to prevent infections in weakened muscles, analgesics for pain, moderate exercise and a nutritious diet.[63] Treatment of polio often requires long-term rehabilitation, including occupational therapyphysical therapy, braces, corrective shoes and, in some cases, orthopedic surgery.[44]

Portable ventilators may be required to support breathing. Historically, a noninvasive, negative-pressure ventilator, more commonly called an iron lung, was used to artificially maintain respiration during an acute polio infection until a person could breathe independently (generally about one to two weeks). Today, many polio survivors with permanent respiratory paralysis use modern jacket-type negative-pressure ventilators worn over the chest and abdomen.[64]

Other historical treatments for polio include hydrotherapyelectrotherapy, massage and passive motion exercises, and surgical treatments, such as tendon lengthening and nerve grafting.[16]

Sister Elizabeth Kenny's Kenny regimen is now the hallmark for the treatment of paralytic polio.[65]

DJ One goal for early action has to be limiting further spread ! But also the best possible care may limit further risks...

I do expect more countries may find polio-virus in sewage water...It is good they communicate that finding...so GP's/hospitals know there may be people with Polio-symptoms...

DJ...IF CoViD does decrease immunity one would expect a lot more cases of Polio. Not only in children but also in adults...Most cases will have NO symptoms, some "flu-like"...but a small group will develop paralysis....So far 1 case did so in the US/New York (20 y/o male) and 1 3 y/o in Israel...

So ...a big question on what most likely billions of CoViD-cases do mean for human immunity...Does it decrease only part of the immunity-giving room for some diseases-not others ?

ALBANY, N.Y. (AP) — State health officials in New York are warning of expanding “community spread” of the polio virus after it was found in wastewater samples from another upstate county.

The state Department of Health said Friday the polio virus was detected in four samples from Sullivan County, two each in July and August. Sullivan County is several dozen miles northwest of Rockland County, where officials on July 21 announced the first case of polio in the United States in nearly a decade. The unidentified young adult was unvaccinated.

The Sullivan County samples are genetically-linked to the case of paralytic polio in Rockland County.

State Health Commissioner Dr. Mary T. Bassett again urged residents to make sure they are immunized, saying “one New Yorker paralyzed by polio is already too many."


“The polio in New York today is an imminent threat to all adults and children who are unvaccinated or not up to date with their polio immunizations," Bassett said in a prepared release.

The virus has now been identified in wastewater samples in three contiguous counties north of New York City: Rockland, Orange and Sullivan. The polio virus also has been found in New York City sewage.

Officials have said that it is possible that hundreds of people in the state have gotten polio and don’t know it. Most people infected with polio have no symptoms but can still give the virus to others for days or weeks.

Polio was once one of the nation’s most feared diseases, with annual outbreaks causing thousands of cases of paralysis. The disease mostly affects children.

People are the only reservoir
Hundreds of diseases can be transmitted between insects, animals and humans. One of the things that makes polio eradicable is the fact that humans are the only reservoir. No poliovirus has been found to exist and spread among animals despite repeated attempts to document this.

DJ, The CoViD pandemic did bring a decrease in polio vaccinations (a.o. in Pakistan, Afghanistan where polio is a main problem). 

Immunization with polio vaccine interrupts virus transmission
Not only are there two safe and effective polio vaccines, but vaccination against polio generates herd immunity, which increases the percentage of the population that is immune to the disease.
